If you wear glasses, you will need them with you. If you wear contact lenses, you will need to be able to remove them on the day and still get home safely. This may mean bringing fluid with you and if necessary, spare lenses or glasses. We cannot complete the eye test if you are unable to remove your lenses.

You will need to bring any prescription medicines in the labeled box or you can bring your repeat prescription or a printout from your GP.

Weighing yourself in KG prior to attending and having your height in CMs can also make your appointment run more smoothly.

For Taxi or Private Hire Medicals, please ask your GP to print out your medical history and bring it with you to your appointment.

Please make sure you have your D4 form or Taxi medical form with you at your appointment.
