Our Medicals are delivered at A1 Roadcraft, Livestock Market, Netherhampton Road, Salisbury, SP2 8RH
Our friendly doctor will complete your medical, including eye test, and fully complete your D4 medical form for you to send off to the DVLA.
Just turn in to the Livestock market site from Netherhampton road and head for the far end of the car park. You will see the entrance from the car park. Head inside and let the team on reception know that you are there for a medical, they will let you know where to wait.
Make sure you bring your D4 or council taxi paperwork for completion. If you do not have a D4 then you can download one from the link below. We complete taxi medicals for lots of different councils so please make sure you bring the correct paperwork with you.
You will need to bring your glasses or contact lenses- make sure if you wear contacts that you can remove them and have either glasses or fluid so that you can see to get home safely!. We will also need to know any medications you take, so bring details.
