 Driver Medical Avonmouth

Our medicals at this location are carried out at Avon Lodge Truck Stop, Third Way, Avonmouth, BS11 9YP

There is onsite parking available.

When you arrive – please head to reception and let them know you are there to Your Driver Medical. They will ask to take a seat and wait for the Doctor to call you through.

What you need to bring to your appointment: 

  • Your forms – If you have booked for a HGV, LGV, Ambulance, Camper, Bus, Coach, PVC, Horse-box or Forklift medical, you will need a D4 form. You can download one here. For TFL (Transport for London) you can download the form you need here. If you have booked a medical for a Taxi/Private Hire licence, you will need to print a copy from you licence providers website or contact your council directly.
  • Identification – This can either be a driving licence or passport.
  • Medications – The doctor will need to know any medication you take at the moment, either long or short term. You can bring a list of medications you take, including the doses, the packs of medication or an up to date prescription.
  • Glasses – You will need to bring glasses if your wear them for driving and an up to date prescription if you have one. If your wear contact lenses, you will need to take the vision test with and without them so please bring spares or glasses with you.
  • Medical History – TFL and Private Hire drivers. Most councils and TFL require us to see your medical history. We do not have access to your medical data so you will need to bring this to your appointment. You must request either a digital or printed copy of your medical history from your GP’s surgery. Please contact us if you need more information.

What to expect during your medical:

  • Our doctor will ask for the relevant forms you need to have filled out by us – see above information
  • The doctor will ask to see some identification.
  • The doctor will ask your weight and height. They can estimate but it’s helpful if you could weigh yourself in KILOGRAMS and measure your height in CENTIMETRES.
  • Your blood pressure will be taken, so the doctor will place the cuff on your upper arm to take a reading – the doctor may need to take several readings during the appointment. Please wear clothing that will allow you to roll up your sleeve to shoulder height or remove a layer whilst maintaining privacy.
  • You will be asked to take a vision test
  • The doctor will complete the form by asking the questions listed.

 

TIPS

  • To help your appointment run smoothly, it’s a good idea to read through your form before you attend the clinic and have the answers ready when you speak to the doctor plus any additional information – do not write on the medical part of the form, the doctor will need to do this.
  • If you are filling out a D2 form along with your D4 form, please read through carefully and check your answers match with what the doctor has recorded. The glasses/vision on the D2 form can be confusing and easily ticked in error.
